Introduction Hospital delirium is a frequent, serious, costly, and underrecognized acute disorder of attention and cognition. Therefore, the prevention of hospital delirium is not only desirable for patients, caregivers, healthcare professionals, and the health system itself, but also a moral duty. Objective To identify and synthetize the main ethical aspects that arise related to the prevention of hospital delirium in patients 65 years and older. Methods A scoping review was carried out in Embase, Medline, and Web of Science. Articles published (...) in English or Spanish analyzing any pharmacologic, non-pharmacologic, or multicomponent intervention for prevention of delirium in hospitalized patients aged 65 years and over were included. Results Twenty-seven studies were included. Based on the frequency of appearance and the relevance, our findings were organized into three main areas. The first one was related to the principle of nonmaleficence and collected findings about the iatrogenic causes of hospital delirium and the use of restraints systems. The second area analyzed the impact of delirium in patients, families, and healthcare professionals from a relational ethics perspective, and addressed the family presence as a factor to prevent delirium. Finally, the third area is related to the quality of care. Conclusion Prevention of delirium in hospitals is essential to avoid the discomfort and potential clinical complications of patients, as well as the suffering of their relatives, caregivers, and healthcare professionals. Policymakers and healthcare managers are encouraged to make the required efforts to prevent delirium in hospitalized older patients. (shrink)

Este artículo tiene como objetivo confrontar la cosmovisión de Octavio Paz, desde la configuración ominosa que le atribuye al hombre y la sugerencia inferida por conllevar su tránsito óptimo de una suspensión o un truncamiento al dinamismo o la concretización de proyectos y utopías. Esa interpretación literaria se desarrolla de «Máscaras del alba» y «Repaso nocturno» de su poemario La estación violenta (1958). Para demostrar esa condensación autoral y comprender su intención, añado la epistemología de Stefano Arduini en función (...) de los campos figurativos que son de utilidad como sistema organizativo de los tropos y que se rigen de pensamientos metafórico, sinecdóquico, metonímico, elíptico, antitético y repetitivo, según cada caso. Esta taxonomía ha sido estudiada por el crítico Camilo Fernández Cozman, quien incorpora para un análisis más fructuoso la intervención de los interlocutores que participan en el discurso lírico. El objetivo de este trabajo es analizar los ejemplos que se dan en las obras tradicionales del denominado καί estructural con el fin de determinar sus características desde una perspectiva más actual. Este es uno de los tipos de καί adverbial que se distinguen, pero, como veremos, se trata de una categoría compleja en la que, sobre la base de su aparición en una oración subordinada, se han mezclado al menos tres usos más o menos habituales de καί adverbial: aditivo, (...) identificativo y corresponsivo. The aim of this paper is to analyse the examples of the so-called structural καί from a modern perspective. The examples are taken from the traditional grammars, in which this adverbial use of καί is usually distinguished. As it will be shown, this is in fact a complex category, established on the basis of its appearance in a subordinate clause and including three quite common uses of adverbial καί: additive, identificational and corresponsive. Este artículo afronta estas dimensiones prestando especial atención al peculiar estilo de Ortega, en particular a las reiteradas metáforas biologicistas, asumiendo que son inseparables del contenido teórico presente en sus obras. Childhood obesity has become a public health epidemic, and currently a battle exists over how to frame and address this problem. This paper explores how public policy approaches can be employed to address obesity. We present the argument that obesity should be viewed as the consequence of a “toxic environment” rather than the result of the population failing to take enough “personal responsibility.” In order to make progress in decreasing the prevalence of obesity, we must shift our view of obesity (...) away from the medical model to a public health model . At the same time, we must be sensitive to the problem of weight bias.Potential obstacles to taking a public policy approach are identified, as well as suggestions on how to overcome them. (shrink)

After years of near total neglect, the problem of childhood obesity is now in the limelight. Terms like “epidemic,” “crisis,” and “emergency” are used frequently when describing the trend. Progress is defined with strong language and fueled by statistics such as the observation that this generation of children will be the first to live shorter lives than their parents. Multi-disciplinary journals such as the Journal of Law, Medicine & Ethics have dedicated symposiums to the issue, and conferences have been convened (...) not only by health professionals but by government agencies, the food industry, and even professionals who market to children. The moral distress of psychologists working in psychiatric and mental health care settings was explored in an interdisciplinary, hermeneutic phenomenological study situated at the University of Alberta, Canada. Moral distress is the state experienced when moral choices and actions are thwarted by constraints. Psychologists described specific incidents in which they felt their integrity had been compromised by such factors as institutional and interinstitutional demands, team conflicts, and interdisciplinary disputes. They described dealing with the resulting moral distress by such means as (...) silence, taking a stance, acting secretively, sustaining themselves through work with clients, seeking support from colleagues, and exiting. Recognizing moral distress can lead to a significant shift in the way we perceive moral choices and understand the moral context of practice. (shrink)

The purpose of professional education programs is to prepare aspiring professionals for the challenges of practice within a particular profession. These programs typically seek to ensure the acquisition of necessary knowledge and skills, as well as providing opportunities for their application. While not denying the importance of knowledge and skills, this paper reconfigures professional education as a process of becoming. Learning to become a professional involves not only what we know and can do, but also who we are (becoming). It (...) involves integration of knowing, acting, and being in the form of professional ways of being that unfold over time. When a professional education program focuses on the acquisition and application of knowledge and skills, it falls short of facilitating their integration into professional ways of being. In addition, through such a focus on epistemology (or theory of knowing), ontology (or theory of being) is overlooked. This paper explores what it means to develop professional ways of being where the focus is becoming, not simply knowing as an end in itself. (shrink)

En este artículo, reviso el concepto y la tipología de violencia condensados por autores como Galtung, Bourdieu, Lacan, entre otros, para fundamentar su existencia en los personajes de La ciudad y los perros y el contexto donde se desenvuelven. La apropiación de ese paradigma de agresión será factible para evidenciar su evolución y su desarrollo humano, porque transitan por un estado de la adolescencia a la madurez. Sin embargo, en ese proceso ontológico, se revela la predominancia de rasgos concomitantes de (...) la ironía que sirven para apaciguar el entorno caótico que les produce permanecer de forma obligatoria en el Colegio Militar Leoncio Prado. Entre las investigaciones de Tenorio, Sobrevilla, Sommers u Oviedo, ya se han corroborado que efectivamente se incluyen patrones de la sátira y la comicidad. Por esa razón, será de utilidad adecuar ejemplos de la novela para demostrar la interacción pertinente de los cadetes en ese microcosmos castrense. (shrink)

Las fake news se suelen caracterizar como contenidos, de modo que prevalece el planteamiento semántico. Aquí se propone un enfoque pragmático, donde son actos comunicativos intencionales. Esto permite entender las fake news desde los actos locucionarios, ilocucionarios y perlocucionarios. Esta triple versión de intencionalidad comunicativa permite una mejor intelección de la mala información (misinformation) y de la desinformación (disinformation), que se refleja con claridad en el caso económico.Para mitigar o neutralizar los daños causados por las fake news en el tejido (...) social, se suele recomendar la contrastación de hechos (fact checking). La contribución de la Economía Experimental —y, en concreto, la aportación de Alvin Roth (Premio Nobel de Economía 2012)— puede ser especialmente relevante en dos direcciones: (a) la actividad económica y (b) la Economía como actividad entrelazada con otras. Esto supone el contrastar hechos partiendo de la intencionalidad en la actividad económica, en cuanto tal y en su nexo con otras actividades humanas. A este respecto, las fake news convergen con la toma de decisiones económicas, en cuanto que ambas están surcadas por la intencionalidad y, además, tienen lugar en un contexto donde adquieren un perfil orientado a fines. In higher education, the conventional design of educational programs emphasises imparting knowledge and skills, in line with traditional Western epistemology. This emphasis is particularly evident in the design and implementation of many undergraduate programs in which bodies of knowledge and skills are decontextualised from the practices to which they belong. In contrast, the notion of knowledge as foundational and absolute has been extensively challenged. A transformation and pluralisation has occurred: knowledge has come to be seen as situated and localized into (...) various 'knowledges', and the status of the body has taken on renewed significance in epistemological debates. Rather than thinking of knowledge as transcending the body, the embodiment of knowledge has become a key factor in understanding the nature of knowledge and what it means to know. In this paper, we adopt a phenomenological perspective in exploring the notion of embodied knowing as it relates to higher education programs and, more specifically, the ways in which information and communication technologies (ICTs) are used in these programs. During the last decades the question of defining life has gained increased interest but, at the same time, the difficulty in reaching consensus on a possible answer has led many to skeptical positions. This, in turn, has raised a wider debate about why defining life is so hard and controversial. Such a debate introduces additional aspects to be considered, like the role and nature of a definition of life itself. In this paper, we will focus on those aspects, arguing that (...) progress can be made if we conceive definitions of life as open heuristic tools that contribute to develop specific research strategies in the biological sciences and, more generally, to increase our understanding of life’s complexity. In contrast with pragmatic or operationalist approaches, we will defend that definitions of life comprise a set of ontological assumptions, together with an inherent unifying vocation, so they should be subject to comparison and critical assessment, closely related to the success or failure of the corresponding research programs, but also to the success or failure in establishing well-grounded interconnections among the latter. We consider that the search for a more coherent, integrated and generalized theory of biology cannot be pursued without keeping an empirical standpoint, and the exercise of defining life should not be taken as an obstacle but as a valuable instrument to achieve that goal. How veridical is perception? Rather than representing objects as they actually exist in the world, might perception instead represent objects only in terms of the utility they offer to an observer? Previous work employed evolutionary modeling to show that under certain assumptions, natural selection favors such “strict‐interface” perceptual systems. This view has fueled considerable debate, but we think that discussions so far have failed to consider the implications of two critical aspects of perception. First, while existing models have explored single (...) utility functions, perception will often serve multiple largely independent goals. (Sometimes when looking at a stick you want to know how appropriate it would be as kindling for a campfire, and other times you want to know how appropriate it would be as a weapon for self‐defense.) Second, perception often operates in an inflexible, automatic manner—proving “impenetrable” to shifting higher‐level goals. (When your goal shifts from “burning” to “fighting,” your visual experience does not dramatically transform.) These two points have important implications for the veridicality of perception. In particular, as the need for flexible goals increases, inflexible perceptual systems must become more veridical. We support this position by providing evidence from evolutionary simulations that as the number of independent utility functions increases, the distinction between “interface” and “veridical” perceptual systems dissolves. Although natural selection evaluates perceptual systems only on their fitness, the most fit perceptual systems may nevertheless represent the world as it is. (shrink)

Scholars have long asserted that public relations (PR) professionals should play the role of organizational conscience, but little research has focused on why and how they play this role effectively. We found that PR professionals who played the role of organizational conscience had broadened conceptions of their roles and responsibilities, including a fervent duty to the public interest. This often put them in the position of providing criticism to powerful organizational players. Rather than raising their ethical concerns as persuasive orators, (...) they used subtle, resourceful, and experiential approaches to persuasion. Playing the role of organizational conscience typically necessitated gaining access to informal coalitions since access to dominant power coalitions often was limited or late. Organizations with participative cultures supported the role of organizational conscience. (shrink)

La crisis de representatividad de los partidos políticos de centro izquierda cuestiona la calidad de la democracia chilena y la proyección de los mismos. No obstante, constituyen referentes sociohistóricos con capacidad de adaptación a los cambios sociales y tendencias de época. Nos interesó conocer cual es el potencial de la generación de relevo de los partidos políticos de adaptarse a los cambios sociales y de representar a la nueva sociedad civil a partir del estudio de su condición generacional dentro de (...) campo político partidista. Los resultados indican que la condición intergeneracional, la visión heterocrítica frente a las formas de reproducción del poder político y la necesidad de legitimidad social favorecen la comprensión de formas distintas de hacer política que lo acercan a la nueva sociedad civil. Las prácticas políticas distintivas identificadas se asocian a la democracia de audiencia, la transversalidad de las alianzas y del territorio. (shrink)

Physical forces regulate numerous biological processes during development, physiology, and pathology. Forces between the external environment and intracellular actin cytoskeleton are primarily transmitted through integrin‐containing focal adhesions and cadherin‐containing adherens junctions. Crosstalk between these complexes is well established and modulates the mechanical landscape of the cell. However, integrins and cadherins constitute large families of adhesion receptors and form multiple complexes by interacting with different ligands, adaptor proteins, and cytoskeletal filaments. Recent findings indicate that integrin‐containing hemidesmosomes oppose force transduction and traction (...) force generation by focal adhesions. The cytolinker plectin mediates this crosstalk by coupling intermediate filaments to the actin cytoskeleton. Similarly, cadherins in desmosomes might modulate force generation by adherens junctions. Moreover, mechanotransduction can be influenced by podosomes, clathrin lattices, and tetraspanin‐enriched microdomains. This review discusses mechanotransduction by multiple integrin‐ and cadherin‐based cell adhesion complexes, which together with the associated cytoskeleton form an integrated network that allows cells to sense, process, and respond to their physical environment. The existence of shoulder abduction (SA) and finger extension (FE) movement capacity shortly after stroke onset is an important prognostic factor, indicating favorable functional outcome for the hemiparetic upper limb. Here we asked whether variation in lesion topography affects these two movements in a similar or a distinct way, and whether lesion impact is similar or distinct for left and right hemisphere damage. SA and FE movements were examined in 77 chronic post-stroke patients using relevant items of the Fugl-Meyer test. (...) Lesion effects were analyzed separately for left and right hemispheric damage (LHD, RHD) patient groups using voxel-based lesion-symptom mapping. In the LHD group, SA and FE were affected only by damage to the cortico-spinal tract in its passage through the corona radiata. In contrast, following RHD, these two movements were affected not only by cortico-spinal tract damage but also by damage to white matter association tracts, the putamen and the insular cortex. In both groups, voxel clusters have been found where damage affected SA and also FE, along with voxels where damage affected only one of the two movements. The capacity to execute SA and FE movements following stroke is affected significantly by damage to shared and distinct voxels in the cortico-spinal tract in LHD patients and by damage to shared and distinct voxels in a larger array of cortical and subcortical regions in RHD patients. Harry Frankfurt's conception of care and love has largely been considered a seductive theory of personality, but an untenable and irresponsible theory of moral normativity. Contrary to that interpretation, this article aims at showing that it is possible to remain faithful to Frankfurt's metaphysical premises while not falling into some moral relativism. First, by comparing Frankfurt's and Heidegger's conceptions of care, I show that Frankfurt's subordination of ethics to carology apparently commits him to a neutral foundationalism. In the next step, (...) I argue that his calling into question of the relation between rationality and morality does indeed subject moral normativity to some subjective and contingent limits. And finally, I show that the objections raised against such a conclusion can be answered by shifting Frankfurt's frontier between contingency and necessity and by exploiting his concept of wholeheartedness. This article offers a philosophical-empirical account of embodied skilful performance in the practice of plant biotechnology. Drawing on the work of Merleau-Ponty and others, we elaborate how skilful performance emerges from and through reciprocal relations encompassing the body-in-the-world and the world-in-the-body. The contribution of this article lies in offering an account of skilful performance that is attentive to a perceiving, motile, feeling body entwined with world. In genetically modifying plants, scientists direct their senses of touch and vision to manipulating plant (...) materials and matter, ‘reading’ subtle changes in tissue cultures, plantlets and so on. In rhythmic movements, they extend their bodies through skilful use of complex equipment, while calling on ‘intuition’ to guide their work. Skilful performance, then, relies on our lived body that is not merely a physical object among other objects, nor a passive instrument for the mind, but dynamic and inescapably entwined with people and things in a world in flux. Bringing to the fore how perceiving, motile, feeling bodies are implicated in skilful performance has significant implications for education and workplace training. (shrink)

El vocabulario en los niños con Trastorno del Desarrollo del Lenguaje (TDL) o con Retraso del Lenguaje (RL) es más pobre que en los niños con un desarrollo típico del lenguaje, por lo que se pretende conocer qué diferencias existen entre ellos. Han participado 66 infantes entre 3 y 5 años; se ha valorado que vocabulario conocen, se ha determinado que tienen menos vocabulario (52%) que sus homónimos (69%), a excepción de la categoría de transportes, en la que tienen los (...) mismos resultados. Finalmente, se determina el listado de categorías que más necesitan trabajar los niños con RL o TDL. (shrink)

The study of agriculture in ancient societies is of vital importance for the understanding of their ecological basis. This article discusses data gathered from Alonso de Molina's dictionary, published in Mexico City in 1571. Molina's information on soil, rain, plants, technology, and human labor applied to agricultural activities gives a picture of the complexity of the several native agricultural systems practiced at that time. Since the Sixteenth Century, native agriculture was impacted by the introduction of new plants, animals, agricultural equipment, (...) and techniques from the Old World. A combination of the two agricultural models conform a new Mexican model of agriculture along the Colonial times. The knowledge of Sixteenth Century agricultural systems is fundamental for the understanding of the history and the conformation of contemporary agro-ecosystems. Recent studies on ecology and social anthropology agree on the necessity to take into account historical data for the discussion of ecological sustainability. (shrink)
